The CMH program helps families of children with special health care needs. CMH works like a back-up insurance for medical services for eligible children under the age of 25 years old in Ohio.

Families on the CMH program are given a Public Health Nurse (PHN):
A PHN is a registered nurse who works with children and families in the community.
PHNs talk with families through phone calls, text messages, emails and home visits.

Provides free services to your family, including:
Explaining the CMH program application and other paperwork.
Helping you learn your child’s medical treatment plan.
Finding you CMH providers for needed medical visits.

Also helps with:
Finding and referring you to community resources.
Helping you learn about your child’s growth and development.
