Midwest Integrated Care provides primary care (sick visits, preventative care, and chronic disease management), behavioral health (counseling and medication management), and substance use disorder treatment services for ages 12+. Midwest practices integrated, whole-person care, where individuals work with a collaborative care team to become champions of their health and well-being. Same-day and walk-in appointments are available. Open Monday, Tuesday, Thursday, and Friday from 8:00 AM - 5:00 PM, and Wednesday from 9:00 AM - 6:00 PM. Located at 775 W.
