We are a federally qualified healthcare center that has general primary care services, behavioral healthcare, counseling, substance abuse treatment and physical therapy. We coordinate and integrate your care across multiple services. If you don't have insurance, the cost of your care will be based on your income and where you fall on our sliding fee scale.
